Pacific Heritage Network is a risk bearing organization with the mission of providing optimal care for underserved and vulnerable communities through partnerships with health plans, health care centers, and health care providers.

Our mission is to expand comprehensive healthcare through a vertically integrated system with a vast network of providers and technology enhanced medical processing systems.

We are an affiliate of Central Neighborhood Health Foundation, a non-profit system of community federal qualified health centers that have served the local neighborhoods of Los Angeles County and the Inland Empire since 1967.
