BACKGROUND
Pacific Heritage Network is an affiliate of Central Neighborhood Health Foundation, a non-profit system of community federal qualified health centers that have serve the local neighborhoods of Los Angeles County and the Inland Empire since 1967.
It is our primary objective to minimize disparities in health care access and outcomes by providing superior-quality, patient-centered healthcare through an integrated delivery system for underserved and vulnerable multiethnic communities.

Pacific Heritage Network is a risk bearing organization with the mission of providing optimal care for the vulnerable population through partnerships with health plans, health care centers, and health care providers.
Our comprehensive network of care and clinically integrated solutionswill enable our patients to receive a seamless uninterrupted medical treatment plan.
